MEDIA CONSENT FORM:
I, hereby give, assign and grant to NCDR, LLC and all practices operating under a practice management agreement with NCDR, LLC and doing business as Kool Smiles (collectively, “Kool Smiles”), the right and permission to use and/or publish my statement, photographs, voice recording, videotape and/or digital images (the “Content”) for media relations use in addition to promotion or education materials that may include but not limited to print publications or materials, electronic publication or presentation and web sites. I agree that my name and identity may be revealed in descriptive text or commentary in connection with the Content. All negatives, positives, prints, digital reproductions and video/audio tape shall be the property of Kool Smiles.
I acknowledge that I will not receive, nor will I be entitled to receive, any compensation or consideration for the use, sale, publication or reproduction of the Content, now or at any time in the future. In addition, I shall have no claims against Kool Smiles (including, without limitation, claims based upon invasion of privacy, defamation, or right of publicity) arising out of any use or alteration of the Content. I further release and hold harmless Kool Smiles, its agents, employees, nominees, assigns, and designees, from any liability arising out of the use, sale, publication or reproduction of the Content.
I was a walk in one afternoon and the people at Kool Smiles worked me in in about an hour, which was amazing considering how busy things were. I guess when you do AWESOME work, a lot of people will come. Dr. Mark and the rest of the staff were very kind and made me feel comfortable. He actually took the time to talk to me about my options and explained to me what he was doing as he did it. I wholeheartedly recommend Kool Smiles to anyone who wants a great experience in a comfortable environment with kind helpful people.
We decided to get our 15 month old son his first dentist appointment with Kool Smiles after reviewing several different places. I was surprised at all of the positive things people had to say and today when we took our son, I found out why! The staff was VERY nice to both my husband and I, as well as our son. Of course, our son didn’t like having strangers looking at his teeth, but the staff was still very gentle and kind to him (yaaay), and extremely patient. The whole experience only took about 10 minutes tops, and when we were done, he got a balloon, AND a toy rubber duck (he LOVES ducks!)… We are very happy! Thanks Kool Smiles!
My children love going to the dentist now! However, I had not yet had a chance to make it to my first appointment with them that was several weeks away and I broke a tooth. I called the office as soon as they opened and explained I had not yet been seen in their office yet and my first appointment was weeks away. They told me to come in immediately and they would work me in and handle things as quick as they could. They fixed my broken tooth, repaired an old cavity (for cosmetic reasons) and were friendly about it all! I was in there and out of there in less than 2 hours as a walk in NEW patient needing work done! I love this place!!
